📣 Eddie's death was avoidable

It was caused by failures at the Trust to recognise his changed feeding plan, the importance of his feeding plan, the fact that getting it wrong could kill him, and the failure to read his notes and communicate translated as effective shedding of responsibility by nursing and clinical staff for dietary and SALT care.  Wes Streeting and the government talk of a 10 year plan for the NHS and of digitizing everything to give patients a voice, but patients like Eddie do not have a voice unless their needs are accommodated and their families and carers are listened to.  It is essential as well for all patients that notes and records are and different carers and clinical staff have effective communication systems, if there are failures of systems patients die or suffer serious harm and we see this time and again in cases like Eddie's but also across the NHS.   The partnership between Fieldfisher and Mencap - Rachel’s Voice - was the means by which Mencap was able to offer Eddie’s family legal support in the lead up to inquest and at the hearing. Without that support we would not have got a verdict of neglect nor drawn the coroner’s attention to the failures in care at the Trust. This partnership continues to offer support to Mencap families.
