The drink most likely was created in the 1970s by George Bullington. It's a simple and easy drink to build. Just Kahlua, creme de cacao, cognac, coffee and cream. After making this several times, I might prefer this to an Irish Coffee. In fact, I think every single one I've come across on a recipe blog is "Keoke." But Keoke does not appear to be a real Hawaiian word or name and if you look at the historic press clippings and the advertisements run by Bully's they spelled it "Keoki" which also matches the Hawaiian spelling.
